I’m excited to share that Arizona Foundational Medicine is officially beginning a soft opening. While the physical clinic space is being finalized, I will begin seeing patients via telemedicine so that those who would like to start care do not have to wait.

During this initial phase, all visits will be conducted through secure telemedicine and messaging. Once the clinic build-out is complete, in-person visits will begin as well.

How to Get Started

1. Submit your information via the form on this page

You will receive an invitation text to the Spruce app, which is the HIPAA-compliant platform we will use for:

• Secure texting

• Video visits

• Phone calls

• Ongoing communication

Once you receive the invitation, please download the Spruce app and complete setup. All communication with the clinic will occur through Spruce.

2. Complete New Patient Registration
Once your reply is received, I will personally set up your profile within our electronic medical record system and send you a secure invitation to join the clinic.

You will also receive your onboarding documents through this system. These are required prior to your first visit (I know it's a pain, I appreciate your time in completing these). All forms can be completed electronically—there is no need to print or fill anything out by hand unless you prefer to do so. Let me know if that's the case and I will send all the attachments. 

 

Once your paperwork has been completed, reviewed, and your dues have been paid, you will be invited to schedule your first visit at your convenience.

What to Expect During the Soft Launch

During this initial phase I will be offering:

• Telemedicine visits

• Medication management

• Chronic disease management

• Lab and imaging ordering and interpretation

• Preventive care consultations

• Care coordination
   

As the clinic build-out is completed, we will expand to include in-person visits, diagnostics, procedures, an on-site gym, weight loss programs, hormone programs, and additional services.
